What to expect from your visit?
- Thorough evaluation of your clinical history with assessment of symptoms and risks
- Counseling on treatment options (hormonal and non-hormonal) with routes and dosages reviewed, evidence-based discussion on risks and benefits of treatment options, and present and historical significance of evidence-based medicine regarding menopause care.
- Personalized treatment plan with resources any necessary lab or imaging orders
Most patients can achieve improvement within the first few months. Most patients can also obtain treatment options with or without insurance coverage of combined medications totaling less than $50/month from retail pharmacies. Most patients do not require additional labwork unless specifically indicated since most menopause/perimenopause management is guided by listening to symptoms and response to treatment.

Does any of this sound familiar?
Women have estrogen and testosterone receptors in every organ system...so symptoms can be any or every organ system.

*Average age of perimenopause? Age 39
*Average age of menopause (defined as no period for 12 months)? Age 51
*Most common symptom of perimenopause/menopause? Joint pain and sleep disruption
*Some of the benefits of HRT? Up to a 60% reduction in heart disease, dementia, and osteoporosis. 30% reduction in colon/lung cancer. Reduction in all-cause mortality with average life expectancy increased by 3.5 years.
*What are the systemic HRT options? Oral, transdermal patches/gel/spray/vaginal ring, and subcutaneous estradiol. Oral/vaginal/rectal progesterone (and other versions of synthetic progestins). Oral, topical gel, and subcutaneous testosterone.
*When can I start HRT? The timing theory shows us that treating symptoms earlier helps prevent chronic disease and relieve symptoms. Treatment is guided by management of symptoms.
*Do I need to wait until menopause for treatment? No. It is safe and beneficial to start treatment when symptoms start.
*Does HRT cause breast cancer? No, in fact, oral equine estrogen showed at 23% reduction in breast cancer and HRT reduces the risk of mortality by 40% if a woman develops breast cancer while on HRT.
*I have a clotting disorder/I have had a blood clot. Can I have HRT? While systemic HRT does not change your increased baseline risk of a clot, topical/subcutaneous estradiol doe not increase your overall risk of a clot and actually have a lower relative risk compared to no HRT. Oral HRT is a relative contraindication.
Am I too old for HRT or menopause treatment? No. While the WHI study showed some increased risks beyond age 70 with new start HRT, route matters with safety and there can still be benefits especially for bone/muscle mass, energy, joints, sleep, UTI prevention. This is an important individualized conversation.
*Can I have HRT if I have had breast cancer? While this is a nuanced, individualized conversation, many patients with a history of breast cancer can have HRT and certainly menopause treatment options that are non-hormonal. Nearly all patients with breast cancer can have local vulvar estradiol which actually decreases all-cause mortality and improves quality of life.
